• This state-of-the-art remote monitoring technology enables our clinical team to continuously track vital signs and ECG data in real time, providing a comprehensive and dynamic view of each patient’s health status. Traditionally reserved for acute care hospitals, VIOS telemetry now allows us to bring hospital-level monitoring directly into our skilled nursing facility.
• By integrating this system into our post-hospital discharge care protocols, we are able to detect early signs of clinical deterioration, enabling faster, more effective interventionsthat help prevent hospital readmissions and support safer, smoother recoveries. It bridges the critical gap between hospital discharge and full recovery with enhanced clinical oversight and patient safety.
• As part of our interdisciplinary care team, our Board-Certified Cardiologistprovides specialized cardiac expertise directly within our facility. Our cardiologist works closely with patients, families and clinical staff to develop personalized treatment plans, monitor cardiac health and ensure timely interventions when needed.
